FOIA Requests
FOIA requests to the U.S. Department of Energy, National Nuclear
Security Administration Service Center may be submitted in writing,
by fax, or e-mail. You may contact the U.S. Department of Energy,
National Nuclear Security Administration Nevada Site Office at the

Requests may be faxed to (505) 845-5063 or submitted electronically using the
FOIA Request form.
Clearly mark the envelope, fax, or e-mail "Freedom of Information Act request."
When submitting a written request, indicate in your letter that you
are making the request under the FOIA. Describe the records you are
requesting as completely and accurately as possible, and provide your
name, address, and a phone number. A telephone call between the
requester and the FOIA Officer can often help clarify a request and
expedite processing time.
Fees may be charged in accordance with the provisions of the FOIA. If
a request will result in fees of less than $15, there will be no fee
assessed. If you wish to address the category of requester you are, or
request a fee waiver, please do so in your initial request letter, as
this will save time in the FOIA process. For a full description of fee

Radiation Exposure History Forms
We often receive FOIA requests for copies of Form NV-192,
Request for Report of Radiation Exposure History
[ PDF, 54 KB ]. It is not
necessary to invoke FOIA to obtain the Form; we will provide the
Form and a stamped, self-addressed envelope it to any requesters
who call or write to our office. Surviving relatives will also
be required to submit an
Affidavit of Surviving Relative Form
